I got it only once. On the day of latest update. Never got it again after it, may be because I clicked "No thanks".

I got it just after my last game with sf. I'm pretty sure its common after the update.

Once. Funny thing was that the "teammate" I had to evaluate was on the other team.

got it 3 times so far so I could verify that it is bugged.

got it 3 times and everytime it gave me an enemy to evaluate even tho it says teammate evaluation
