^ this is your answer. Make stacks with 2 camps.

Stack the small camp because you get those groups of 4 or 5 creeps in there and no group is smaller than 3.

Otherwise just wait till late game. I got it on Medusa about an hour into the game when the creepwaves were huge. I just walked down mid, cleared 2 waves and suddenly I had it.

Heroes like AM, Meepo and Naga who can either be in 2 places at once or move around really fast are best for this. The difficulty isn't in actually killing the creeps, its getting enough of them in the same place.
